Overview

• Role: Registered Nurse (RN) - PRN
• Comp: $70.50 per point
• Mileage in addition to productivity comp
• Schedule: Part-Time (Schedule based on availability) minimum of 10 points per week.
• Location: Field-based
• Carry a high-quality caseload: Ensure visit quality, with OASIS accuracy that holds up to review.
• Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
• Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
• Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.
• Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
• Direct, high-acuity patient care, with a focus on wound care
• Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases.
• Being the face of Adaptive to patients, families, and the referral sources who feed your pod.
• Dedicated clinician: home health experience strongly preferred
• High ownership: you treat your caseload like your own panel and your pod like your own business.
• Open mindset: Energized by the opportunity to shape the playbook and refine SOPs
• Tech-forward: You want to leverage software to accelerate the charting process, and you'll give us blunt feedback to make it better

• Active Michigan RN license
• Reliable transportation, valid driver's license, auto insurance
• Home health / OASIS experience strongly preferred
• $1k bonus for referrals (we’re growing)!
• Mileage reimbursement
